PAYROLL INFORMATION FOR CLASSIFIED (NON-TEACHING) SUBSTITUTES
 
Please completely fill out the linked forms. These are required by various Federal and State agencies for all new hires.
These forms include:
1. SERS Enrollment Form
2. Federal Withholding Form
3. Ohio Withholding Form
4. Ohio New Hire Form
5. I-9 Employment Eligibility Form
6. Authorization for Direct Deposit
7. Industrial Commission Form
8. Public Employment Form

All payments will be made through Direct Deposit. A copy of your current Driver’s License and Social Security Card must be submitted to the Treasurer’s Office before payment can be made.
 
Due to new Medicare Tax Laws: The W2 Annual Wage Statement and your CHECK will be issued as the name appears on your Social Security Card. If your Social Security Card name is not correct, you may contact the local Social Security Office to have the name changed. Once the Treasurer’s Office receives a copy of the new card; your name will be changed
on our records.

1. STRS New Hire Notification
2. STRS Statement Concerning Social Security
3. Re-employed Retiree Notifivation STRS (use only if retired from Ohio Public Retirement System)
4. Federal Withholding Form
5. Ohio Withholding Form
6. Ohio New Hire Form
7. I-9 Employment Eligibility Form
8. Authorization for Direct Deposit
9. Industrial Commission Form
10. Public Employment Form

All payments will be made through Direct Deposit.

If your are receiving Benefits from State Teachers Retirement System (retired member) please contact the Treasurer’s Office. STRS has a different enrollment form for Retirees.
